Job Description:

We are seeking an Expert End-of-Life Care Specialist to join our team at Heart of Hospice, LLC.As a Hospice RN Case Manager, you will be responsible for delivering high-quality patient care and developing trusting relationships with patients and their families.Key Responsibilities:

• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Identify patients' physical, psychosocial, and environmental needs, and re-assess as needed, no less than every 15 days.
• Initiate and coordinate the plan of care, maintaining accurate and relevant clinical notes regarding the patient's condition.
• Collaborate with patients, families, attending physicians, and other healthcare professionals to provide comprehensive care.Requirements:
  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR Certification.
  • Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.Additional State-Specific Requirements:
