Job Description:
A company is looking for a Remote MTM Pharmacist to deliver
telephonic medication therapy management services.
Key Responsibilities
Deliver telephonic medication therapy management (MTM) services to
improve member care
Perform medication reconciliations with high-risk members and
communicate medication-related issues
Develop member-specific treatment plans based on nationally
recognized guidelines
Required Qualifications
Bachelor's degree in pharmacy or PharmD from an accredited school
of Pharmacy
Active pharmacist license in HI, TX, IL, FL, or PA
Previous pharmacy experience delivering MTM preferred
Basic computer skills and proficiency with Microsoft Office
products
Experience in a work from home environment
