Job Description:
A company is looking for an RN Population Health to provide
clinical and operational coordination of Risk Adjustment
efforts.
Key Responsibilities
Serve as a subject matter expert in Risk Adjustment Factor (RAF)
tools and coordinate their implementation
Establish collaborative relationships with physicians and
third-party vendors to communicate clinical information
Analyze and monitor clinical Risk Adjustment reports for accuracy
and compliance with CMS requirements
Required Qualifications
Current, valid RN license in the state of practice
3-5 years of related healthcare experience
Proficiency in personal software applications, including word
processing and spreadsheets
Experience with healthcare provider relations
Understanding of Medicare Advantage funding models and Risk
Adjustment processes
