  • RN Case Management/Discharge Planning position at University Hospital in Jackson, Mississippi.
  • Must have at least 2 years of Acute Care Management experience in discharge planning.
  • Experience must include at least 6 months at a level 1 or large academic Medical Center hospital.
    • Job Requirements
    • BLS certification required.
    • RN Case Manager certification required.
    • CCM preferred but not required.
    • EPIC charting experience is required.
      • Schedule Information
      • 5x8 hour shifts, non-negotiable.
      • Monday to Friday, 8:00 AM to 4:30 PM.
      • No weekend requirements, holiday requirements rotated as needed.
        • Unit Specific Information
        • Department: CM - General Case Manager/Discharge Planning.
          • Additional Details
          • FLU vaccine is required at UMMC with only medical exemptions allowed.
          • Fingerprint requirement: Remote process must allow 10 days, in-person can be done at Jackson or Grenada campus the week prior to start.
          • Must have experience in medical discharge planning.
